C-3D Triumphant Transitions: Reimagining the Reentry Process (CEU) Track: Juveniles/Social Work NEW! Room: National Harbor 5 This session will assist participants in developing the infrastructure needed to ensure a more seamless transition of youth from the facility into community-based resources that will meet their varied needs. This will include creating Transition Adjustment and Planning Sessions (TAPS) which will match, link and integrate transitioning youth into the community-based systems and services that will meet their psychological, educational, social and familial needs in an effort to improve outcomes and maintain their placement in the community. Participants will become apprised of available community resources that are federally funded and nationwide which can assist their clients and discover strategic means to meet the
