C-3H Coordinating Complex Community Transitions for Reentry (CE) Room 128B Track: Healthcare The South Carolina Department of Corrections (SCDC) customized the current comprehensive Reentry Program to create a multidisciplinary, multiagency collaboration to address complex release planning for returning citizens with special needs; Intellectual Disability, Serious Mental Illness, Aged and infirmed — needing Long-Term Care, etc. Involvement of many federal, state, and community agencies in pre-planning discharge is integral to success of reentry, to include workgroup meetings, communication networks, benefits enrollment for services ranging from housing to a medical/mental health/SUD care to obtaining credentials. Complex transition is a multi-faceted process and requires the cooperation of many individuals and disciplines both within SCDC and within the State of South Carolina. Learning Objectives:
