Reentry in San Diego Room 1B Under the leadership of the San Diego County Probation Department, the Community Transition Center (CTC) has picked up over 12,000 clients from the California Department of Corrections and Rehabilitation and transported them to the center for assessment and linkage to services. Co-located at a substance use disorder program, the CTC is the location of a Multi-Disciplinary Team comprised of probation officers, mental health clinicians and drug treatment staff. Clients can stay at the center if needed. The presentation will focus on how front-end assessment-based case planning can reduce recidivism and why advocacy is critical to client care. Lastly, how cross-system collaboration can positively impact clients and the community. Learning Objectives: • Participants will learn how cross system collaboration is critical to client success. • Participants will be provided tools they can implement in their local jurisdictions to increase effective reentry practices. • Participants will be able to describe the San Diego CTC model and how it has positively impacted client care.
