Written policy, procedure, and practice require that the institution have com munication with the attending health care providers regarding the inmate’s health care concerns and supervision requirements. Communication between the facility and primary community referral center regarding inmate care should occur at least annually. Comment : Communication between facility and primary community referral center provider should occur to assist with security and public safety. Sometimes communications may be required for specific inmates. Protocols : Written policy and procedure; Agreement; Memo of Understanding. Process Indicators : Communication records between facility and primary com munity referral center; Health records; Interviews. Inmates who need health care beyond the resources available in the facility, as determined by the responsible health care practitioner, are transferred un der appropriate security provisions to a facility where such care is available. Comment : Treatment of an inmate’s condition should not be limited by the resources and services available within a facility. Health care staff should col laborate with security personnel in determining conditions of transportation and necessary security precautions when an inmate needs to be transported to another facility or provider. Protocols : Written policy and procedure; Referral consult form. Process Indicators : Health records; Completed referral consult records; Transportation logs; Interviews. A transportation system that assures timely access to medical services that are only available outside the correctional facility is required. Such a system needs to address the following issues: 1. Prioritization of medical need. 2. Urgency (for example, an ambulance versus a standard transport). 3. Use of a medical escort to accompany security staff, if indicated. 4. Transfer of medical information. The safe and timely transportation of inmates for medical, mental health, and dental clinic appointments, both inside and outside the correctional facility (for example, hospital, health care provider, or another correctional facility) is the joint responsibility of the facility or program administrator and the health services administrator.
