Statistical Reports 5-JCF-5C-39 (Ref 4-JCF-4C-38)
Health care statistical reports are prepared quarterly and include, at a min imum, the use of health care services by category, referrals to specialists, medication usage, laboratory and x-ray tests completed, convalescent admis sions (if applicable), hospital admissions, serious injuries, illnesses, suicide at tempts, deaths, and off-site transports Comment : Statistical reports assist in the monitoring of health care services and can be used to help justify the allocation of resources.

Internal Review and Quality Assurance 5-JCF-5C-40 (Ref 4-JCF-4C-39)
A system to review and monitor health care services is developed and imple mented by the health authority The necessary elements of the system shall include an annual review by the health authority of each policy and procedure in the health care delivery system and revision, if necessary It also shall include a review and evaluation of medical data, including health- records Such health-records should contain information on the following: 1 Prescription and medication administration practices 2 Complaints and grievances 3 Outcome measures and statistical reports 4 Contagious-disease management 5 Suicide attempts or suicides 6 Deaths The following steps should be part of the review: 7 Developing and implementing of a multidisciplinary quality- improve ment committee 8. Creating of corrective-action plans based on the findings to address and resolve identified problems and concerns, including educational and train ing activities 9 Re-evaluating problems or concerns to determine whether the corrective measures have achieved and sustained the desired results 10 Issuing a quarterly report to the facility or program administrator of the findings of health care review activities. Comment : Evaluating data and adherence to corrective action plans should result in more effective access to services, improved quality of care, and better utilization of resources. Health care review activities shall comply with confidentiality and legal requirements.
