A health care treatment plan shall be developed for juveniles who require medical supervision for chronic and convalescent care This plan includes di rections to health care providers and other facility personnel regarding their roles in the care and supervision of the juveniles This plan is approved by the responsible health care authority Comment : Juveniles who may require treatment plans include the following: the chronically ill, juveniles with serious communicable diseases, the physically dis- abled, pregnant juveniles, and the terminally ill. Protocols : Written policy and procedure. Process Indicators : Completed treatment plans; health-record entries; observa tions; interviews. Juveniles with chronic illnesses or conditions, such as asthma, diabetes, and other diseases, receive periodic care and treatment that includes: 1 Medication monitoring 2 Laboratory testing 3 Specialist consultation, as needed 4 Health care practitioner review and examination, as indicated Comment : Age-specific medical reference materials are available. Protocols : Written policy and procedure. Process Indicators : Health records, to include medication monitoring, laboratory testing, and specialty consultation; completed physical examinations and reviews; health care treatment plans; observations; interviews. (MANDATORY) Therapeutic diets are prescribed for juveniles by a health care practitioner, as necessary A therapeutic-diet manual is available in the health-services area for reference and information Comment : Prescriptions for therapeutic diets should be specific and complete, fur nished in writing to the food service manager, and rewritten at least quarterly. Therapeutic diets should be kept as simple as possible and should conform as closely as possible to the foods served to other juveniles. Protocols : Written policy and procedure; diet manual. Process Indicators : Completed therapeutic diet orders; completed therapeutic diet records; health-record entries; observations; interviews.
