10 Body deformities and ease of movement 11 Condition of the skin, including trauma markings, bruises, lesions, jaundice, rashes and infestations, recent tattoos, and needle marks or other indications of drug abuse Medical disposition of the juvenile: 12 Cleared for general population 13 Cleared for general population with a referral to appropriate health care service 14 Referral to appropriate health care service for emergency treatment When juveniles are referred for emergency treatment, their admission or return to the facility is predicated on written medical clearance Comment : Health screening of all admissions is necessary to detect juveniles who pose a health or safety threat to themselves or others from being admitted to the general population and to identify juveniles who require a referral or immediate medical attention (for example, juveniles, who are unconscious, semiconscious, bleeding, or otherwise obviously in need of immediate medical attention). When juveniles are referred to an emergency department, their admission or return to the facility is based on written medical clearance. Protocols : Written policy and procedure; training curriculum for health-trained personnel. Process Indicators : Completed screening forms; admission logs; staff training re cords; health-record entries; observations; interviews. (MANDATORY) All intrasystem transfer juveniles receive a health screen ing by a qualified health care professional or health-trained personnel, which commences on their arrival at the facility. All findings are recorded on a health screening form approved by the responsible health care practitioner in cooperation with the health authority When health-trained personnel con duct the health screening, procedures shall require a subsequent review of positive findings by a qualified health care professional. At a minimum, the health screening shall include at least the following: Inquiry into: 1 Whether the juvenile is being treated for medical or dental problems 2 Whether the juvenile is presently on medication 3 Whether the juvenile has a current medical or dental complaint
