Comment : The case plan of each juvenile must be clearly written and in compli ance with agency timelines. The plan is to be individualized and specific. The expected accomplishments must be measurable and achievable and agreed upon by both the juvenile and staff. Goals shall be reviewed on an ongoing basis and re vised when deemed necessary by the staff and/or juvenile. Changes in the plan are to be discussed with the juvenile and accompanied by the signature of the juvenile and the staff.

Relevant information and programming efforts in regard to juveniles will be communicated and established through a coordinated team approach Each juvenile will be reviewed at least monthly The outcome of these meetings shall be documented Comment : It is essential a team of staff involved in the juvenile’s daily living are involved and have input in the planning process of the juvenile. Adjustments and changes in the juvenile’s program and plan are to be reflected in the team minutes and in the individual case plan, when necessary.

Individual Contacts 5-JCF-6C-07 (Ref 4-JCF-5C-07)
Social services staff members are to be available to meet with juveniles on a regularly scheduled basis at least once a month and at the juvenile’s request Individual contacts are documented in the form of an individual contact note in the juvenile’s record Commen t: Individual meetings/counseling sessions can assist juveniles with per sonal problems and adjustment difficulties. Staff shall make scheduled appoint ments for juveniles on a regular basis and at their request.
