Summary-Admission Report 5-JCF-6A-02 (Rev 4-JCF-5A-02)

There is a summary admission report prepared for all new admissions to the system that includes but is not limited to the following: 1 Summary account of the legal aspects of the case 2 Summary of criminal history 3 Family history 4 Medical, dental, and mental health and substance abuse histories 5 Vocational interests, if appropriate 6 Educational status 7 Religious background and interests 8 Recreational assessment 9 Psychological evaluation, including intellectual assessment The summary admission report is updated as needed upon transfer or recom mitment Comment : The summary admission report constitutes the first document in the juvenile’s cumulative case record. This document may be duplicated and used for review by the classification committee and the unit management team. It is an essential tool for the development of a program for the juvenile, including reentry preparation. Programming During Reception 5-JCF-6A-03 (Ref 4-JCF-5A-03)

Juveniles are provided with programming, including education, during the reception period New juvenile offenders receive written orientation materials and/or translations in their own language if they do not understand English When a literacy problem exists, a staff member assists the juvenile in under standing the material Comment : The daily program during the reception process should include inter- views, testing, and other admissions-related activities, including distribution of information on programs and services. New juvenile offenders should be pro- vid ed reading materials, be permitted to attend religious services, receive exercise on
