Mental-Health Evaluation 5-JCF-5D-04 (Ref 4-JCF-4D-04)

Juveniles referred for a mental health evaluation and/or mental health treat ment will receive a comprehensive evaluation by a qualified mental health care professional The evaluation is to be completed within 30 days of the

referral request date and includes at least the following: 1 Review of mental health screening and appraisal data 2 Review of the individual’s mental health history 3 Direct observations of behavior

4 Collection and review of additional data from individual diagnostic in terviews and tests, as appropriate, assessing personality, intellect, and/or coping abilities 5 Recommendations for treatment with appropriate referral to include transfer to a specialized unit or appropriate mental health facility when psychiatric needs exceed the treatment capability of the facility or agency Comment : Individual psychological evaluations should be performed when there is a reasonable expectation that such evaluation will serve a therapeutic or dis positional function useful to the overall interests of the juvenile. Written reports describing the results of the assessment should be prepared, and all information should be appropriately filed.

Process Indicators : Health records; completed referral forms; interviews with mental health provider(s).

Mental-Health-Treatment Plans 5-JCF-5D-05 (Ref 4-JCF-4D-05)

A mental health treatment plan shall be developed for juveniles being treat ed on an ongoing basis by a qualified mental health care professional. The plan will be developed within 30 days of initiation of treatment and revised as needed Treatment plans will include juvenile participation to the extent that is possible Comment : Sound treatment planning for juveniles with mental health needs in cludes participation by the juvenile to the extent that the juvenile is able to par ticipate in developing the mental health treatment plan and coordination with the juvenile’s treatment team.
