Qualified Health Care Staff – see health care professional or professional staff or health care practitioner/ provider.

Qualified Medical Person – see health care professional.

Qualified Mental Health Person – see mental health care practitioner/provider/professional.

Qualified Mental Health Professional - see mental health care practitioner/provider/professional

Quality Assurance – formal, internal monitoring program that uses standardized criteria to ensure quality and consistency. The program identifies opportunities for improvement, develops improvement strategies and monitors effectiveness.

Quality Assurance Program – see quality assurance.

Rated Capacity – the original architectural design plus, or minus, capacity changes resulting from building additions, reductions, or revisions. Reasonable Accommodation – modifications or adjustments, which enable qualified applicants with dis abilities to access the job application process or which enable qualified employees with disabilities to per form the essential functions of the job and to enjoy the same terms, conditions, and privileges of employ ment that are available to persons without disabilities. Reasonably Private Environment – this may vary, depending on individual and institutional circumstanc es, but it is one which will maintain the dignity of the disabled individual in light of that person’s disability. Records (Juvenile and Adult Offenders) – information concerning the individual’s delinquent or crim inal, personal and medical history and behavior, and activities while in custody, including but not limited to commitment papers, court orders, detainers, personal property receipts, visitors’ lists, photographs, fin gerprints, type of custody, disciplinary infractions and actions taken, grievance reports, work assignments, program participation, and miscellaneous correspondence. Referral – process by which a juvenile or adult offender is introduced to an agency or service that can provide the needed assistance. Refocus Area - a designated place where brief voluntary separation, from the general population may oc cur. (Refocus Rooms replace the calming, safe and time out areas, and comfort room). Release – residents released from a community corrections facility is inclusive of all program close-outs to include but not limited to: removal, program failure, absconders, sentence time served, and successful completions. Release on Bail – release by a judicial officer of an accused individual who has been taken into custody on the accused’s promise to appear in court as required for criminal proceedings.
