Navigating the Quality Roadmap [CE|CME|CERP] Room 102 C Primary Community of Focus: Adult Corrections Overview: The Institute for Health Care Improvement has developed the Model for Improvement which is considered now as the framework for doing any quality improvement project. The presentation will cover in detail how to formulate an effective aim statement, how to construct the different key metrics (Outcome Measure, Process Measure, and Balancing Measure) and will provide an introductory overview of run chart to better understand the different variations in the processes over time. The Model for Improvement ensures teams know the purpose behind what they are trying to accomplish, understand what success will look like and identify those changes that will result in improvement. This presentation will assist the audience to appreciate QI. Learning Objectives: • Explain how to effectively formulate an AIM statement utilizing the AIM statement builder.

Children with incarcerated parents are six times more likely to become incarcerated themselves, highlighting the urgent need to preserve and strengthen family bonds during custody. The Bonding, Empowering, and Reuniting (BEAR) Families Program promotes family connectedness and reunification through an innovative curriculum, structured activities, video visits, and contact visits. BEAR is one of the few contact visitation programs flexible enough to serve individuals in the general population and mental health housing. This presentation reviews the BEAR model and shares key implementation challenges and successes.
