In community settings, Primary Care– Mental Health Integration (PC-MHI) advances health span through early identification of behavioral health needs, preventive engagement, and reduced medical utilization. This session explores how integrated care principles including systematic screening, stepped care, shared treatment planning, and coordinated medical-behavioral collaboration can be adapted for correctional settings. Through case examples and standards alignment, participants will learn practical strategies to reduce crisis-driven care, improve continuity, strengthen suicide prevention, and support prevention-focused, team-based correctional healthcare. This workshop explores the application of these tools and processes through the Gateway Foundation Internal Evaluation Process. It will identify application of the tools to the individualization of client care, application to program evaluation, and assessment of service efficacy. The presentation will identify how this effort has enhanced program effectiveness through the use of empirical data to drive program design.
