This workshop presents the evolution of the Reentry Success Services Program at the Weber County Correctional Facility (WCCF) in Utah, a boundary-spanning, data-informed, and partnership-driven model designed to improve public safety and reduce recidivism. Beginning with just two staff members and limited in-reach services in 2019, the program has grown into a multidisciplinary initiative supported by federal funding (COSSUP), community coalitions and formalized cross-agency information sharing and coordination of care. As a result of this program and reentry work being done at WCCF, Weber County Sheriff’s Office was the recipient of the “Innovation at an Accredited Facility Corrections Award” at the August 2025 ACA Congress of Correction in Denver, CO. Participants will learn how Weber County built a reentry system rooted in evidence-based practices, including the Risk-Need-Responsivity (RNR) model, trauma-informed care, peer recovery support and Medication-Assisted Treatment (MAT). The session will highlight key milestones such as the formation of the Weber County Community Reentry Coalition, implementation of screening and assessment tools, development of boundary-spanning positions and “warm handoffs” to community based treatment and resources, and the creation of the Weber CARE (Coordinated Alignment for Reentry & Engagement) model of information sharing and care coordination.

Presenters will candidly discuss implementation challenges — including culture change within corrections, staffing limitations, interagency coordination and gaps/barriers to access of treatment and needed resources such as housing and transportation — and the lessons learned through iterative program development. The workshop will also present outcome data demonstrating significant reductions in recidivism and system utilization, along with strategies for sustainability, including alignment with Medicaid policy changes and future program expansion. With increasing national focus on behavioral health, opioid use disorder, and justice system reform, this workshop provides a replicable model for correctional agencies seeking to improve outcomes while maintaining safety, accountability and fiscal responsibility. This session is designed for correctional leaders, program managers, policymakers and practitioners seeking actionable strategies to build or enhance reentry systems that are collaborative, scalable, and responsive to complex populations.
