It is a necessary evil in healthcare and is often bemoaned by providers due

to the time-consuming task it has become. However, it is one of the most important pieces of healthcare; and often the least emphasized in our educational process and clinical training. The intention of this workshop is to discuss the necessary elements of a clinical note, reinforce importance of good notations for several reasons including inter-disciplinary communication, patient safety and care, auditing/data collection and risk management. Learning Objectives: 1.Participants will be able to define the necessary components of a chart note. 2.Participants will be able to discuss the importance of good record keeping. 3.Participants will be able to identify a poorly constructed clinical note versus a well written note.
