Retford , Practice Leader, HOK Architecture, Salt Lake City, Utah C-2G Transition from Jail to Community Initiative: Designing a Systemic Flow for Special Populations in Programming Room 208B Track: Jails This presentation will focus on the critical nature of assessing individuals for their needs at the time of incarceration. Assessed planning is conducted to involve the individual in the appropriate programs that address their needs while at the same time make community placement connections prior to release. Initiated in 2009, Transition from Jail to Community Models make appropriate placement and connections at a critical point for each individual. Community partner development of relationships prior to release is shown to be the key for continued connection

upon release. During the pandemic, this model continues to exist and move forward due to the partnerships we had forged over the years. Learning Objectives: • Learn how using a Proxy Score can help sort participants into the appropriate programming for whatever criminogenic needs they show. • Learn how to use and share Needs Assessment tools with participants and partners. • Learn how connecting participants with community partners while incarcerated will increase those connections upon release.
